Education amplified the divide: young women whose highest credential was a high-school diploma were nearly twice as likely to have skipped screening (around 39 percent) as those holding a master’s or doctoral degree (approximately 17 percent). Insurance status compounded the problem for older women; nearly 32 percent of uninsured women aged 30 to 65 had never been screened, versus 11 percent of those with private coverage.

Why Untreated HPV Becomes Cancer

More than nine out of ten cervical cancers originate from a persistent infection with high-risk strains of human papillomavirus, a virus transmitted through routine intimate skin-to-skin contact. In most people the immune system clears the virus without lasting effect. In a smaller subset, however, the infection endures and drives abnormal changes in cervical cells. Wen flagged several important caveats. Because screening history was self-reported, respondents may have misremembered whether or when they were last examined. The survey also cannot establish causation: researchers observed that among women aged 30 to 64, rates of later-stage cervical cancer climbed by more than two percent per year after 2017, and that women 65 and older likewise saw rising rates of more advanced disease. Those trends are alarming, but the data cannot confirm that reduced screening is the driver rather than a parallel symptom of broader access problems.

Possible explanations for the decline span a wide range — inadequate insurance coverage, geographic or financial barriers to care, discomfort with the procedure itself, and simple unawareness about when screening should begin and how often it should repeat. Disentangling those factors requires further research.

How Screening Actually Works

Two principal methods form the backbone of cervical cancer screening. The Pap test collects a small sample of cells from the cervix using a spatula or brush, then examines those cells under a microscope for precancerous or malignant changes. HPV testing, by contrast, detects the genetic material of high-risk HPV strains directly, often through a vaginal swab. Depending on age and prior results, a clinician may recommend one test, the other, or both at defined intervals.
